THIS IS A DROP-OFF PARTY. I understand that this is a drop off party, and non-painting adults with the party is limited to up to 2 parents/chaperones. You are welcome to paint separately in our open studio if capacity allows or take a stroll and enjoy one of the many great shops on Main Street. The party guests will all be seated together in a private party room and a Crackpots host will be helping to guide the party. The party is 1.5 hours.

I will not send my child to this party if my child is sick or showing symptoms. I understand that Crackpots follows BCPH recommendations and I agree to follow the rules that are currently in place.

I release, hold harmless, defend and indemnify Crackpots, Inc, its employees & officers from any and all damages, injuries, exposure, claims and causes of action for participation in this activity..

I certify that I am the guardian of the child(ren) listed as participants on this form. By sending my child to this party, I accept the terms & conditions set forth by Crackpots.
